- Membership dues will be automatically charged to the members credit card on file on the date agreed upon when creating the membership.
- Monthly memberships are non-transferable and may not be shared.
- Monthly membership payments, or if paid in full, is non-refundable.
- The Membership is a 12-month commitment that begins of the date of purchase. Credit card on file is required for monthly charges.
- You are responsible to ensure that the credit card number on file with LB+W is valid so that the monthly fee can be charged to that card. You may change the credit card provided at any time. It is your responsibility to make sure that there is always a valid credit card number on file with LB+W for payment of the Monthly Fee. If there is not a valid credit card on file or the card declines at the time the Monthly Fee is charged to that card and one is not provided within ten days after the regularly scheduled date for payment, LB+W may exercise any and all rights available to it, whether under this agreement or under applicable law, which may include, but are not limited to, suspension of your membership in the membership until such payment is made, assessing a late payment fee of $25.00, and/or terminating this Agreement.
- LB+W Gift Cards may not be redeemed to pay any portion of the membership fee. The membership fee cannot be combined with any other promotion and/or discount.
- LB+W reserves the right to vary, add, or eliminate any of the services provided from time to time.
- LB+W reserves the right to close or vary facility hours without notice.
Membership Term
The initial membership period shall be for a period of three months (the “Initial Period”). A member shall not be entitled to terminate or suspend their membership during the Initial Period. If the contract is terminated before the expiration date, there is an early termination fee of $100.00 that will be charged to the card on file. Any contract that has been terminated prior to the expiration date will have a waiting period of 1 year before being able to reinstate.
After the Initial Period, the membership shall automatically renew monthly. A 30-day written notice before your next scheduled payment is required to cancel a monthly membership plan. No refunds will be issued after monthly membership due has been charged or paid in full. Cancellation requests must be emailed to hello@liftedbeautyandwellness.com.
Freezing a Membership
There may be times when a member will need to freeze their membership for a short period of time, due to circumstantial reasons such as military leave, pregnancy, extended illness, relocation, etc. When a membership is frozen, the member is not authorized to use their membership services or benefits during the freeze period. You must be a member for at least 3 months and have made at least 3 month’s membership payments to be eligible to freeze your account. Memberships may be frozen once during your annual contract period, for a minimum of 30 days and a maximum of 3 months, depending on the circumstantial reason that will be determined by LB+W.
Notice to Members
Do not sign this agreement before you have read it in its entirety. A copy of this agreement will be available on the member’s client portal. The member acknowledges that this document is an agreement and will become legally binding upon its acceptance by Lifted Beauty & Wellness (LB+W). The member also understands there will be no refunds issued for any charged member dues. The undersigned member acknowledges receipt of LB+W membership terms and conditions and has read, understands, and agrees to be bound by the terms and conditions as part of this agreement. Any member, who is under the age of 18, must have a parent or legal guardian co-sign and be present during all office visits. The co-signer, along with the member, agrees to be bound by all terms and conditions of this agreement.
Release and Waiver of Liability
I have read and understand this waiver and have been fully informed of all of LB+W membership terms and conditions as well as membership benefits and limitations. I certify that I have disclosed all medical conditions that might affect my treatments. I understand that LB+W providers cannot diagnose any medical conditions and release them from any injury resulting from undiagnosed medical conditions present during my treatment. I assume all responsibility for updating changes in physical and mental condition and for reporting all injuries sustained at LB+W at the time of service.

LB+W is not responsible for any injury or loss of property to any person while on the premises or participating in LB+W services. As a member, I assume full responsibility for services received at LB+W and shall indemnify LB+W, its affiliates, agents, independent contractors, and employees against any and all liability arising from services rendered.
