1. Included in our package is lab testing including a gut health test from Genova Diagnostics along with either a DUTCH hormone evaluation or Organic Acids Test from Mosaic Diagnostics. 
2. The Hormone Test (either DUTCH or OAT) and Genova Gut Health Test are included in your package. 
3. Additional bloodwork may be recommended by Dr. Fox and will be ordered through LabCorp. This is not included in your package price but can typically be billed to insurance, depending on your coverage. 
4. All acne services are eligible for FSA/HSA payment. 
5. Clients are responsible for submitting out-of-network reimbursement claims, if desired.
